Program Eligibility


Program Eligibility:  

  1. the applicant is a student who has been diagnosed with Attention Deficit Hyperactivity Disorder, Autism Spectrum Disorder, or a Learning Disability by a licensed physician or registered psychologist or candidate-registered psychologist. The above professionals must have expertise and training in making these diagnoses.
     
  2. the applicant has secured a placement in a designated special education private school  or a registered special education private school for the upcoming or current academic school year;
     
  3. the applicant has previously studied in a public school in Canada;
     
  4. the applicant has received educational programs and services in a public school setting under an individualized program plan (IPP) established in accordance with policies made by the Minister or has documented evidence that the decision has been made through the program planning process to proceed with the development of an individualized program plan;
     
  5. the applicant has received confirmation from the designated or registered special education private school that he/she will receive individualized programs and services, in the form of an IPP and/or Adaptations while at the DSEPS/RSEPS and;
     
  6. the applicant has submitted a written rationale explaining how the programs and services to be provided by the designated special education private school will positively affect their academic and behavioral well-being.

 

Required Documentation Checklist
Diagnosis
  • AD/HD - Assessment document(s) and/or letter from diagnosing physician detailing the diagnosis
     
  • ASD - Assessment document(s) and/or letter from diagnosing physician detailing the diagnosis
     
  • LD - Psycho-educational assessment supporting the diagnosis as per the LDAC definition, 2002.
DSEPS/RSEPS
  • Acceptance letter from the DSEPS/RSEPS
     
  • Copy of the proposed Adaptation and/or Individual Program Plan
Study in Canada
  • Report card from most recent public school
IPP
  • IPP report attached (Academic, Life Skills, Enrichment or Social Development) or
  • Letter from the school principal, or designate, indicating that the program planning process has reached Stage 4 and based on the student's strengths and challenges in relation to the learning outcomes, the decision has been made to develop an IPP.
Rationale Statement
  • A written rationale prepared by the applicant explaining how the programs and services referred to in their application documents will benefit their academic and behavioral well-being. 
     
  • A parent/legal guardian may apply on behalf of a student under the age of majority.
